Primary Care Neighborhood, Coast of Bays, Location to be determined
This position will support the Primary Care Neighborhood (PCN) in the Coast of Bays. A Primary Care Neighborhood is a geographic area where all community based primary healthcare providers function as an interconnected team in delivering the care patients require. As a member of the PCN, the successful candidate will work collaboratively to provide team based comprehensive, patient centered, and culturally safe healthcare to meet the needs of patients and communities. PCNs will enable the delivery of a range of quality care for patients within their own communities while facilitating interdisciplinary work amongst healthcare providers.

Definition of work:
The Nurse Practitioner is responsible to provide comprehensive assessment and care to clients. The successful applicant works independently in diagnosing conditions, prescribing medications, performing or ordering diagnostic tests and treatments, evaluating treatments and providing follow-up care in liaison with the interdisciplinary team. The Nurse Practitioner provides comprehensive and holistic nursing care to clients including health promotion, injury and illness treatment and prevention, supportive, curative and rehabilitative care in a primary care practice setting.
As an advanced practitioner, the Nurse Practitioner functions in the roles of practitioner, educator and leader. As governed by provincial regulations, he/she practices independently, collaborating with the primary care physicians in the area for issues outside the Nurse Practitioner scope of practice. This position may also require travel and the utilization of virtual technologies in supporting the clientele within the neighborhood. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
* Graduation from a Nurse Practitioner program approved by Central Health.
* Current registration as a Nurse Practitioner with the Association of Registered Nurses of Newfoundland and Labrador.
* Current BLS certification.
* Demonstrated knowledge of home and community health services.
* Demonstrated exceptional leadership qualities and problem-solving skills as well as oral and written communication skills.
* Demonstrated ability to work well independently and in an interdisciplinary team environment.
* A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
* Consistent demonstration of Central Health's corporate values; Compassion, Accountability, Respect, Stewardship and Quality
* A satisfactory record of work performance and attendance is required for this position.
* A satisfactory Certificate of Conduct from the RCMP or local police authority.
Hours of work:
75 hours biweekly (Shift work may be required)
